Habitus und Personalmix in der stationären Langzeitpflege : eine praxeologisch-wissenssoziologische Studie.

  • Abstract Hintergrund Ein seit langem bekannter Pflegepersonalnotstand in der stationären Langzeitpflege verlangt nach neuen Kooperationsformen zwischen Pflege, Sozialarbeit (≙ Betreuungsarbeit) und Hauswirtschaft, mit neu zu justierenden Aufgabenbeschreibungen und Arbeitsneuverteilungen. Damit ein optimaler multiprofessioneller Skill- und Grade-Mix in der stationären Langzeitpflege gefunden werden kann, ist es notwendig, die derzeit bestehende Logik der Praxis auf der Mikroebene zu erforschen. Nur so können mögliche Ressourcen, kreative Potentiale, aber auch destruktive Handlungsprinzipien sichtbar gemacht werden. In diesem Zusammenhang spielt das handlungsleitende Wirken der Erfahrungsräume „Profession“ und „Migration“ eine große Rolle. Beide Erfahrungsräume stellen eine reale Quelle zur Anhebung verfügbarer Arbeitskräfte dar und sind bei einem wirksamen Neuausrichten eines multi-professionellen Personalmix zu berücksichtigen. Abstract Methode Anhand von neun episodischen Interviews nach Flick und drei Gruppendiskussionen nach Bohnsack mit Fachkräften der Altenpflege, Pflegehilfskräften, Personen der Betreuungsassistenz und der Alltagsbegleitung wurden mittels der dokumentarischen Methode von Bohnsack jene Prozessstrukturen rekonstruiert, die den Alltagsroutinen der Praxisakteur*innen in der multiprofessionellen Zusammenarbeit als (unbewusste) handlungsleitende Prinzipien zugrunde liegen. Mit Hilfe der formulierenden und reflektierenden Interpretation wurden sinngenetische Typen gebildet, aus denen hervorgeht, mit welchen unterschiedlichen Orientierungen (Habitus) Akteur*innen ihre spezifischen Themen bzw. Probleme bearbeiten. Um die sozialen Zusammenhänge aufklären zu können, innerhalb derer die empirisch gewonnenen Typiken stehen, wurden diese anhand ihrer ihnen innewohnenden sinngenetischen Typen systematisch mit den als Basistypiken fungierenden Erfahrungsräumen „Profession“ und „Migration“ in den Vergleich gesetzt. Abstract Ergebnisse Es konnten 18 sinn- und 8 soziogenetische Typen rekonstruiert werden. Einen Zusammenhang zu den Erfahrungsräumen „Profession“ und „Migration“ weisen Typen mit fehlender oder geringfügiger Berufsqualifikation und Sprachbarriere auf. Diese Typen stehen in einer multiprofessionellen Abhängigkeit und zeigen eine multiprofessionelle Überforderung. Dabei ist die typische Handlungsorientierung auf das Umsetzen des gegenwärtigen modus operandi ausgerichtet. Einen Zusammenhang zum Erfahrungsraum „Profession“ zeigen Typen mit dreijähriger Berufsausbildung. Diese präsentieren eine multiprofessionelle Souveränität. Die typische Handlungsorientierung besteht hierbei in der Weiterentwicklung des gegenwärtigen modus operandi. Abstract Zusammenfassung und Fazit Durch die Methoden der Datenerhebung und der dokumentarischen Methode von Bohnsack konnten sinngenetische Typen (thematische Handlungsorientierungen der Akteur*innen) und soziogenetische Typen (Wirkzusammenhänge zu den Erfahrungsräumen „Profession“ und „Migration“) rekonstruiert werden. Die unbekannte Logik der sozialen Praxis konnte partiell aufgezeigt werden. Vorhandene Ressourcen im Sinne einer qualitativen und quantitativen Ausführungsverantwortung, kreative Potentiale im Sinne einer Fürsorge- und Führungsverantwortung sowie destruktive Handlungsprinzipien im Sinne eines physischen und psychischen Macht-Ungleichgewichts konnten partiell aufgedeckt wer-den. Konstruktive Schritte für eine Weiterentwicklung der stationären Langzeitpflege können dementsprechend zielführender gegangen werden. Forschungsdesiderate sind sichtbar geworden. Abstract: Background A nursing shortage in patient long-term-care that has been known for a long time calls for new forms of cooperation between nursing, social work (≙ care work) and housekeeping, with task description that need to be readjusted and work redistributed. In order to find an optimal multi-professional skill an grade mix in inpatient long-term care, it is necessary to research the current logic of practice at the micro-level. Only in this way can possible resources, creative potential, but also destructive principles of action be made visible. In this context the action-guiding effect of the experience spaces „profession“ and „migration“ plays a major role. Both areas of experience represent a real source for increasing the available labor force and must be taken into account in the effective reorientation of a multi-professional personal mix. Methods On the basis of nine episodic interviews according to Flick and three group discussions according to Bohnsack with geriatic care professionals, nursing assistants, people in care assistance and everyday support, those process structures were reconstructed using Bohnsack’s documentary method, (unconscious) action-guiding principles. With the help of the formulation and reflective interpretation, meaning-genetic types were formed, which show the different orientations (habitus) with which actors work on their specific topics or problems. In order to be able to elucidate the social context within which the empirically obtained typologies stand, they were systematically compared with the experiential spaces “profession” and “migration”, which acted as basic typologies, on the basis of their inherent meaning-genetic types. Results 18 sensory and 8 sociogenetic types could be reconstructed. Types with missing or minor professional qualifications and language barrier show a connection to the experience areas “profession” and “migration”. These types are in a multi-professional dependency and show a multi-professional overload. The typical action orientation is geared towards the implementation of the current modus operandi. Types with three years of vocational training show a connection to the experience area “profession”. These present a multi-professional sovereignty. The typical action orientation consists in the further development of the current modus operandi. Summary and conclusion Using the methods of data collection and Bohnsack’s documentary method, it was possible to reconstruct sensory-genetic types (thematic action orientations of actors) and socio-genetic types (causal connections to the experiential spaces “profession” and “migration”). The unknown logic of social practice could be shown partially. This means that existing resources in the sense of a qualitative and quantitative responsibility for implementation, creative potential in the sense of a care and leadership responsibility as well as destructive principles of action in the sense of a physical and psychological power imbalance could be partially uncovered. Accordingly, constructive steps for further development of inpatient long-term care can be taken more purposefully. Research desiderata have become visible.
