Patienten, die ein schweres Schädel-Hirn-Trauma oder eine intrakranielle Blutung erleiden, bedürfen meist einer intensivmedizinischen und komplexen pflegerischen Versorgung. Dabei besteht insbesondere bei neurochirurgischen Intensivpatienten auch über die Akutphase ihrer Erkrankung hinaus ein hohes Risiko für Komplikationen. Durch den Einsatz von Advanced Practice Nurses mit einer Spezialisierung auf neurochirurgische Intensivpatienten wird es möglich, diesen vulnerablen Patienten eine erweiterte und vertiefte pflegerische Versorgung zukommen zu lassen.
Background: An ever-increasing number of patients seek health information via the internet. However, there is an overabundance of differing, often low-quality information available, while a lack of health literacy makes it difficult for patients to understand and assess the quality and trustworthiness of the information at hand. The web portal tala-med was thus conceived as an evidence-based, up-to-date, and trustworthy information resource for lower back pain (LBP), which could be used by primary care physicians (PCPs) and patients during and following consultations for LBP. The current evidence demonstrates that patients with LBP could benefit from web portals. However, the use of such portals by patients remains low, thus limiting their effectiveness. Therefore, it is important to explore the factors that promote or hinder the use of web portals and investigate how patients perceive their usability and utility.
Objective: In this study, we investigated the acceptance, usability, and utility of the web portal tala-med from the patient perspective.
Methods: This qualitative study was based on telephone interviews with patients who had access to the web portal tala-med from their PCP. We used a semistructured interview guide that consisted of questions about the consultation in which patients were introduced to tala-med, in addition to questions regarding patient perceptions, experiences, and utilization of tala-med. The interviews were recorded, transcribed, and analyzed through framework analysis.
Results: A total of 32 half-hour interviews were conducted with 16 female and 16 male patients with LBP. We identified 5 themes of interest: the use of tala-med by PCPs during the consultation, the use of tala-med by patients, its usability, added values derived from its use, and the resultant effects of using tala-med. PCPs used tala-med as an additional information resource for their patients and recommended the exercises. The patients appreciated these exercises and were willing to use tala-med at home. We also identified factors that promoted or hindered the use of tala-med by patients. Most patients rated tala-med positively and considered it a clear, comprehensible, trustworthy, and practical resource. In particular, the trustworthiness of tala-med was seen as an advantage over other information resources. The possibilities offered by tala-med to recap and reflect on the contents of consultations in a time-flexible and independent manner was perceived as an added value to the PCP consultation.
Conclusions: Tala-med was well accepted by patients and appeared to be well suited to being used as an add-on to PCP consultations. Patient perception also supports its usability and utility. Tala-med may therefore enrich consultations and assist patients who would otherwise be unable to find good-quality web-based health information on LBP. In addition, our findings support the future development of digital health platforms and their successful use as a supplement to PCP consultations.
Treatment thresholds and minimal clinically important effect sizes of antiosteoporotic medication

Background
Patient decisions to take preventative treatments for osteoporosis depend on their perceptions of fracture risk, medication effect sizes (ES) of benefits and harms. However, physicians and lay persons may have differing perceptions of risks and medication efficacy. Both tend to overestimate medication benefits. This study surveyed at what risk physicians would prescribe and lay persons would be willing to take bisphosphonates, the minimum ES both groups do demand and the physicians estimates of the actual benefit of bisphosphonates.
Design
Cross-sectional online questionnaire survey.
Methods
Respondents were confronted with a case vignette with an osteoporotic patient (10-year femoral fracture risk: 32%). They were asked at what threshold of 10-year-risk of femoral neck fracture they would prescribe or take a drug. They were asked for the minimum ES (absolute risk reduction, ARR) they demand from bisphosphonates to prescribe or take them. Physicians were asked to provide their estimate of the actual ARR of bisphosphonates.
Results
114 physicians and 140 lay persons answered (convenience sample/snowball distribution). The 10-year-risk threshold of lay persons (Mdnlay = 60%) willing to take medication was twice as high as the physicians’ threshold (Mdnphy = 30%) to prescribe it (p < .001). The median minimum ARR physicians demanded for bisphosphonates prescription was 17%, whereas lay persons demanded 22% (p < .001). Physicians estimated the actual ARR of bisphosphonates to be 12%. This estimated effect size was below their own minimum threshold for prescription.
Conclusions
Lay persons tolerate a higher fracture risk and demand a larger benefit of antiosteoporotic medication for fracture prevention than physicians. Physicians demand higher minimum benefits than their own estimates which in turn are above the benefit evidence suggests. Physicians should be more familiar with ES of antiosteoporotic drugs concerning patient outcomes and actively advise lay persons before preventive treatment decisions are taken.
Vor dem Hintergrund der aktuellen demografischen Entwicklung spielen chronische
Erkrankungen eine zunehmend größere Rolle. Eine dieser chronischen Krankheiten ist die chronische Herzinsuffizienz. Um damit zu leben, müssen Betroffene im Rahmen der Therapie oftmals große Bereiche ihres Lebens einer Veränderung unterziehen. Das Ziel der vorliegenden Arbeit ist es, am Beispiel einer an chronischer Herzinsuffizienz erkrankten Person Faktoren zu eruieren, die diesen Prozess unterstützen.
Um dieser Forschungsfrage nachzugehen, wurde ein qualitatives, narratives Interview mit einer Person geführt, die vor mehreren Jahren die Diagnose HI erhalten hat. Der Krank-heitsverlauf und der Umgang der betroffenen Person mit ihrer Situation wurden daraus rekonstruiert, inklusive der Gründe für das Gelingen oder Versagen der Therapie.
Als größter förderlicher Einflussfaktor konnte das soziale Umfeld der Person ausgemacht
werden. Das zeigt, dass das soziale Umfeld einer Person eine wertvolle Ressource für
gesundheitsförderliches Verhalten sein kann.
Bei zukünftigen Therapieplanungen ist zu empfehlen, die eruierten Ressourcen und positiven
Einflussfaktoren zu nutzen.
Objective:
This thesis deals with the phenomenon of 'pyjama paralysis', meaning the influence of nightwear on the mobility and activity of acute inpatients. It tries to identify the phenomenon with scientific findings, to classify it in terms of nursing care and to describe it based on the available literature.
Introduction:
While outpatients concentrate on going home, patients who are assigned a bed develop a psychological resistance against leaving the hospital. They sink into the "comfort of the hospital community", where they are not expected to do anything for themselves or make their own decisions. Experts call this condition pyjama paralysis. During a hospital stay, a person spends most of the day sitting or lying down and usually alone, without using much energy. Almost two thirds of inpatients wear nightwear or pyjamas during the day. The majority of lying patients wear nightwear, with three out of five lying elderly patients having no medical reason for bed rest.
Inclusion criteria:
The inclusion criteria were limited to adult patients capable of mobilization in an acute inpatient setting using the Pico scheme (JBI). The phenomenon of interest was termed pyjama paralysis and refers to the relationship between the wearing of pyjamas and activity and mobility.
Methods:
A scoping review was performed to get an overview of the topic. For this purpose, a systematic search in the subject databases CINAHL / EBSCO, PubMed, OPACplus / EBSCO, ProQuest, Research Gate / Chadwyck Healey, Wiley and the electronic database of the Lenoir - Rhyne - University (LRU) Library was performed. The full text search covered a period of ten years (2009 - 2019). The search was not restricted linguistically and was completed on 09.01.2020.
The results:
It was possible 25 articles and 12 qualitative and quantitative studies could be included. The present overview contains the nursing relevant aspects of the phenomenon pyjama paralysis clustered in categories and applicable to the nursing process. The phenomenon of pyjama paralysis is characterized by the wearing of nightwear or hospital shirts, outside of regular sleeping and resting hours, in an acute inpatient setting, which is associated with a predominantly inactive (sitting / lying down) daily routine and behavior that is not medically justified.
In the acute inpatient setting, this phenomenon occurs in about 40% of patients over 65 years of age. The factors influencing the activity are mainly psychological. The influence on
dignity, self-confidence and well-being is based on the external effect and symbolism of the garment, which promotes devitalization. Environmental influences are also decisive. Devitalization is accompanied by a decrease in mobility and agility, which has extensive physical and psychological effects and involves the risk of iatrogenic impairment and complications. The studies favors the education and information of patients, their environment and nursing staff, as well as the function-focused continuation of daily activities and mobility measures tailored to the patient in an activity-enhancing and safe environment, in order to counteract pyjama paralysis and positively influence care-sensitive outcomes.
Conclusions:
The phenomenon of pyjama paralysis is an influencing but also directly influenceable factor in this context. The care-relevant aspects of pyjama paralysis are linked to ethical, psychological and physical aspects. Superficially, the solution to the enormous problem of inactivity of inpatients appears simple if it is reduced to changing the clothing style. In reality, it is very complex and characterized by the influence of several factors, which all interact and are interdependent. Therefore, future research on the topic of pyjama paralysis should focus on systemic rather than individual interventions.
Hintergrund: Schlaganfallpatienten sind anfällig für Deliren, dass negativen Auswirkungen auf die Mortalität, Morbidität, Verweildauer im Krankenhaus und Outcome zur Folge hat. Obwohl es ein lebensbedrohliches psychisches Syndrom ist, gibt es bisher keine evidenz-basierte Empfehlung für die routinemäßige Früherkennung, die die neurologischen Defizite aufgrund eines Infarktes einbezieht.
Zielsetzung: Das Ziel dieser Arbeit liegt darin, vorhandene Delir-Screenings auf die Eignung für Schlaganfallpatienten unter Berücksichtigung der neurospezifischen Herausforderungen zu vergleichen und die diagnostische Testgenauigkeit zu überprüfen.
Methodik: Eine systematische Literaturrecherche angelehnt an Robert Shaw (2019), wurde nach den Richtlinien für bevorzugte Berichtspunkte für systematische Überprüfungen und Metaanalysen (PRISMA) im Zeitraum Juli 2018 bis Dezember 2021 in den Datenbanken Medline, Cinahl und Cochrane Libary durchgeführt.
Ergebnisse: Von 2636 Treffern konnten 18 Studien eingeschlossen werden. Insgesamt 3320 Patienten und acht verschiedene Delir-Screenings wurden einbezogen. Die gepoolte Sensitivität und Spezifität für den 4AT beträgt 82% bzw. 77% und für den CAM-ICU 78% bzw. 90%. Patienten, die entweder durch den CAM-ICU oder 4AT mit Delirium identifiziert wurden, wiesen einen höheren Anteil an neurologischen Defiziten wie Aphasie, Dysathrie, Neglect und Hemiparesen auf. Es besteht ein Zusammenhang zwischen neuro-spezifischen Herausforderungen und einer positiven Delirbewertung mit den Screenings. Der ICDSC beruht auf Beobachtungen und kann demzufolge die neurologischen Defizite zum Teil berücksichtigen.
Schlussfolgerung: Obwohl die diagnostischen Testgenauigkeitsparameter akzeptabel waren, umfasst kein Screening die Überschneidung zwischen neurologischen Defiziten und Symp-tomen im Zusammenhang mit Delirien. Der Beobachtungsscore ICDSC hat Potenzial durch eine Etablierung in Stroke Units auf die komplexen Patientengruppe ausgerichtet zu werden.
Introduction: In emergency care, geriatric requirements and risks are often not taken sufficiently into account. In addition, there are neither evidence-based recommendations nor scientifically developed quality indicators (QI) for geriatric emergency care in German emergency departments. As part of the GeriQ-ED© research project, quality indicators for geriatric emergency medicine in Germany have been developed using the QUALIFY-instruments. Methods: Using a triangulation methodology, a) clinical experience-based quality aspects were identified and verified, b) research-based quality statements were formulated and assessed for relevance, and c) preliminary quality indicators were operationalized and evaluated in order to recommend a feasible set of final quality indicators. Results: Initially, 41 quality statements were identified and assessed as relevant. Sixty-seven QI (33 process, 29 structure and 5 outcome indicators) were extrapolated and operationalised. In order to facilitate implementation into daily practice, the following five quality statements were defined as the GeriQ-ED© TOP 5: screening for delirium, taking a full medications history including an assessment of the indications, education of geriatric knowledge and skills to emergency staff, screening for patients with geriatric needs, and identification of patients with risk of falls/ recurrent falls. Discussion: QIs are regarded as gold standard to measure, benchmark and improve emergency care. GeriQ-ED© QI focused on clinical experience- and research-based recommendations and describe for the first time a standard for geriatric emergency care in Germany. GeriQ-ED© TOP 5 should be implemented as a minimum standard in geriatric emergency care.
Hintergrund: Im Jahr 2014 wurden insgesamt ca. 476.000 neue Krebserkrankungen in Deutschland diagnostiziert. Der Bedarf an Beratung, Nachsorge und Begleitung ab dem Zeitpunkt der Diagnosestellung ist ein wichtiger Grundstein für die weitere Behandlung. Pflegekräfte nehmen hier eine zentrale Rolle ein. Die sogenannten „Onkolotsen“ begleiten die Betroffenen von Beginn an und klären über die Therapiegestaltung, Nebenwirkungen, Ängste und Sorgen auf. Die psychosozialen Bedürfnisse werden erfasst und die Lebensqualität durch erhalten und erkennen von Ressourcen positiv beeinflusst. Das System der „Onkolotsen“ ist ein kontinuierlicher Prozess und wird an die Lebensumstände, Einschränkungen in der Lebensqualität durch Symptomlast der Erkrankung angepasst.
Ziel: Anhand verfügbarer Literatur zeigt dieses integrative Literaturreview den Einfluss von „Onkolotsen“ auf Erwachsene mit neu diagnostizierter Krebserkrankung auf.
Methodik: Es wurde eine systematische Literaturrecherche in den Datenbanken Pubmed, CINAHL, Cochrane Library und Springer Link durchgeführt. Die Recherche fand ab 30. September 2019 bis 20. November 2019 statt. Die Suchstrategie wurde mit Hilfe eines Flussdiagrammes veranschaulicht. Die Ergebnisse werden zusammengefasst dargestellt.
Ergebnisse: Fünf passende Studien wurden beurteilt und die Ergebnisse in dieser Arbeit zusammengefasst. Durch alle einbezogenen Studien wurde deutlich, dass Onkolotsen einen Einfluss auf die Lebensqualität, Zufriedenheit mit der Versorgung und auf die Belastung, Angst und Depression haben. Der positive Effekt auf viele Ebenen im Gesundheitswesen wurde deutlich. Die Kostenstrukturen, Beratungsplanungen und die Nebenwirkungen können besser kalkuliert und gesteuert werden.
Schlussfolgerung: Die Einführung eines „Onkolostensystemes“ in Deutschland wäre wichtig und sinnvoll. Der Umfang der Tätigkeit und die Ausbildung sollte gut geplant und strukturiert eingeführt werden. Es profitieren nicht nur die Betroffenen und deren Angehörige. Alle Personen und Institutionen, die ab der Krebsdiagnose involviert sind, erfahren einen entlastenden und positiven Effekt.
